The Challenge

Jude had difficulty moving his wrist, forearm and fingers with control. His muscle weakness in his whole right upper limb made movements slower and more effortful. Jude’s fingers were all hypermobile and he found it difficult to orientate his fingers around objects. Over the next 3 weeks we wanted to get Jude’s right-hand side as strong and as mobile as we possibly could.

‘He’s able to put his shoes and socks on now which is obviously going to help him at school and help mum in the mornings with getting ready” - David, Jude’s dad.

The Goals

  • Increase range of movements in his thumb, wrist and arm.
  • Strengthen his right upper limb.
  • To be able to isolate his index finger and thumb.
  • To be able to achieve pincer grip.
  • To be able to use cutlery independently.
  • To be able to put his socks and shoes on independently.

The Science-y Bit

Jude had hypermobility in his affected limb which is a problem caused by muscle imbalance. Muscle imbalance in Jude’s case could be reduced through strengthening the hand/wrist flexors and stretching the hand/wrist extensors.

Our physiotherapists aimed to achieve these goals through play-lead sessions with Jude, increasing the use of his affected limb through the cast and constraint of Jude’s well-functioning arm. Jude increased the use of his affected arm/hand as soon as the cast was on and over the course of an intensive 3 week programme; we saw incremental improvements to his hand strength, arm-use and co-ordination and an improvement in general preference to utilise his affected limb.

The Achievements

  • Jude has the ability to point which makes it easier for him to play with his ipad.
  • Jude gained active movement in his thumb which means he is able to give a thumbs up.
  • Jude’s coordination improved which means he can throw and catch a ball.
  • The quality and range of Jude’s grasp improved so he is now able to play with any toys: big or small.
